Clinical assessment of chronic pain in children and adolesecents: recommendations of the working group on pediatric pain of the Spanish Pain Society

ABSTRACT

Introduction: Under the auspices of the Working Group on Pediatric Pain of the Spanish Pain Society, professionals from different health specialties and fields participated in a working group to identify the domains and assessment scales in Spanish languaje that should be included in clinical trials involving children and adolescents whith chronic pain.
Material and methods: The consensus was completed by consulting and incorporating the participants ´comments at a National Conference on Childhood Pain at which these domains and measures were presented. There was a consensus among the investigators that seven domains should be included: pain intensity; physical functioning; emotional functioning; personal or role functioning; adverse effects and events; sleep; and global assessment of satisfaction with treatment. For each domain, those scales with validated version in Spanish and freely available were selected although choosing them with the best psychometric qualities.
Results: The review and the expert consensus made it possible to identify valid, accessible and Spanish-language scales for the seven defined domains.
Conclusions: These recommendations will help Spanish-speaking health care professionals and researchers in sharing data, comparative reporting of results, unification of criteria for the review of manuscripts, and, therefore, it will facilitate decision making by health care professionals with regard to the treatment of children and adolescents with chronic and recurrent pain.



RESUMEN

Introducción: Bajo los auspicios del Grupo de Trabajo de Dolor Infantil de la Sociedad Española del Dolor, profesionales de distintos estamentos y especialidades sanitarias participaron en un grupo de trabajo en el que se identificaron los dominios y las escalas en español de valoración que se aconseja deberían incluirse en los ensayos clínicos en los que se evalúa el dolor crónico en niños y adolescentes.
Material y métodos: El consenso se completó mediante una consulta a los participantes a una Jornada Nacional de Dolor Infantil en la que se presentaron dichos dominios y medidas y en la que se incorporaron sus comentarios. Hubo consenso entre los investigadores en que debían incluirse siete dominios: intensidad del dolor, funcionamiento físico, funcionamiento emocional, funcionamiento personal o de rol, efectos y eventos adversos, sueño y valoración global de satisfacción con el tratamiento. Para cada dominio, se seleccionaron aquellas escalas con mejores cualidades psicométricas que tuviesen una versión validada en lengua española y que fuesen de acceso libre.
Resultados: El trabajo de revisión y el consenso de expertos permitió identificar unas escalas válidas, accesibles y en lengua española para los siete dominios definidos.
Conclusiones: Estas recomendaciones ayudarán al personal sanitario y a los investigadores de lengua hispana, a la puesta en común de datos, la notificación comparada de resultados, la unificación de criterios para la revisión de propuestas de manuscritos, y, por tanto, facilitarán a los profesionales sanitarios la toma de decisiones con respecto al tratamiento de niños y adolescentes con dolor crónico y recurrente.
